Transportation to Wellness Way

Starting the first week in April 2026, construction will begin on the building adjacent to 6 Wellness Way, Latham. You will notice temporary changes to traffic flow and parking for construction. Follow the large posted signs—they’ll guide you where to go. View our visual overview of the detours and traffic patterns that will be in place until Spring 2027.

Wellness Way is conveniently located just off the Northway and Rt. 9 in Latham. CDTA bus route #182 will take you to the Wellness Way – CDPHP/Community Care Physicians stop.

We have maximized the clinical space at Wellness Way for our patients. All Community Care Physicians practices, along with ConnectRx Latham and CDPHP Customer Connect, previously located in the Capital Region Health Park (711/713 Troy Schenectady Rd.) in Latham have moved two miles away to Wellness Way.

ConnectRx Pharmacy

ConnectRx is a full-service onsite pharmacy, powered by CDPHP. We accept most major insurances including NYRx and EPIC. The pharmacy is open Monday – Saturday for your prescription and over-the-counter needs, with free home delivery available. Our pharmacists collaborate with you and your provider to make sure your medications are working their best for you.
